First Visit

Thank you for considering us for your dental needs. 


For all new patients, we recommend setting up an appointment for a comprehensive exam by Dr. Haskell prior to scheduling your cleaning. As all patients have different needs when it comes to getting their teeth cleaned, it will allow us to schedule you appropriately with our hygienist.


If it is determined that you have dental needs other than just a simple cleaning, we will set up your appointment accordingly. 


Please inform our office prior to your first visit if any precautions must be taken for dental treatment (See Below).


Please assist us by providing the following information at the time of your visit:


Any x-rays taken at your previous dental office (bring a copy or email them to rjhaskelldmd@outlook.com).


A list of any medications you are presently taking.


Certain medical conditions or previous surgeries require Antibiotic prophylaxis prior to dental procedures including routine hygiene visits. These conditions include prosthetic heart valves and artificial joints (hips, knees, shoulders, etc.) Prior to your first visit, please check with your surgeon’s office to see if Antibiotic prophylaxis is needed in your circumstance. If the answer is yes, either your surgeon or our office can prescribe the appropriate antibiotic.


If you have dental insurance, please bring your insurance card so we can enter your insurance information and generate a claim for today’s visit. 


Important: A parent or guardian must accompany all patients under 18 at the initial visit.
